Healthier communities
We will work in partnership to build strong neighbourhood-based support, that helps people stay well and reduces avoidable pressure on hospital services.
High level, 5-year ambition
-
Work with partners to develop neighbourhood teams focused on prevention, early intervention and continuity of care.
-
More of our care is delivered outside of hospital settings and we have shifted resources to achieve this.
-
Our services and decision making are driven by population data and insight from our communities.
Patients will feel
-
supported earlier to stay well and avoid unnecessary hospital visits
-
their care is coordinated between teams and settings
-
more choice and control over how and where they receive care
TeamLGT will feel
-
confident in patient-centred partnership working across primary, community, mental health and voluntary services
-
supported to deliver care that improves patients’ independence and prevents patients needing a higher
-
level of care or their conditions from worsening
How will we determine success?
-
Reduce unplanned hospital admissions and stays of one day or over by 10% for priority cohorts by March 2029.
-
Reduce hospital outpatient appointments for children under the age of 16 by 10%.
-
By March 2029, keep growth flat and work towards an overall reduction in nonelective admissions for high priority cohorts.
-
Contribute to an overall reduction in type 1 ED attendances for high priority cohorts.
