Our dedicated multi-disciplinary team provide a comprehensive service for all types of diabetes.

We provide a wide range of services and specialist clinics, including an insulin pump service. Our Community team provides diabetes clinics in the hospital, at community sites in the borough and an outreach service for housebound patients.

Diabetes education

  • Structured Education for Type 2 Diabetes - DESMOND (Diabetes Education and Self Management for Ongoing and Newly Diagnosed Diabetes)
  • General practice support:
  • Expert advice, case discussion and support are available through a practice visit, participation in multidisciplinary team meetings, joint clinics, and advice over the telephone.
  • Education sessions for: GPs, practice nurses, district nurses, community nurses and in-patient staff
  • Diabetes evening forums

How to get a referral

You can be referred by your GP, practice or district nurse. Once we have received your referral we will contact you with appointment date.

About your appointment

When you come to see us, you must bring an up to date list of any medications that you take, as well as any home blood sugar monitoring records if you have them.

If you are currently taking insulin,  you should also bring your insulin pen with you.

Please do try to arrive at least 15 minutes before your appointment.
