About the service
Our Trust provides comprehensive diabetes care for adults, young people and housebound patients across its hospital and community services.
Our teams support people with all types of diabetes through specialist assessment, education, treatment planning and ongoing management.
Services include consultant-led and nurse-led clinics, dietetic support, insulin pump services, young adults’ care, inpatient diabetes specialist nursing support, and community diabetes clinics delivered in hospital, community and outreach settings.
We also support the coordination of digital retinal screening appointments for diabetic eye disease and work with patients to develop personalised care plans, including clinical management plans for young people aged 18-25 under the care of adult physicians.
Care is provided across Queen Elizabeth Hospital, University Hospital Lewisham and community sites in Lewisham.
How to get a referral
Referrals can be made by GPs, district nurses or via outpatients clinics.
Routine new referrals are seen within a few weeks.

Diabetes education
- Structured Education for Type 1 Diabetes (DAFNE) 1 week course and recently established -1 day over 5 weeks
- Structured Education for Type 2 Diabetes - DESMOND (Diabetes Education and Self Management for Ongoing and Newly Diagnosed Diabetes)
- General practice support:
- Expert advice, case discussion and support are available through a practice visit, participation in multidisciplinary team meetings, joint clinics, and advice over the telephone.
- Education sessions for: GPs, practice nurses, district nurses, community nurses and in-patient staff
- Diabetes evening forums
General endocrinology clinics including:
