We need your views on our Community services in Lewisham

At Lewisham and Greenwich NHS Trust, we are working to develop a new plan for the Community health care services we provide in Lewisham. This will set out our ambitions for the future.

We want to understand what people think about these community health services. This short survey gives you a chance to share your views with us, so that our plans reflect what patients want from these services.

Community health services support people in lots of different ways, such as support from district nurses where people live, support from specialist nurses to manage a long term condition, or support to return home after a stay in hospital. You can read more about what these services are on the NHS website. They are commonly used by children, people living with long term conditions and older people.

The services take place in lots of different places, including people’s homes, care homes, health centres, children’s centres, and schools. They might sometimes take place at University Hospital Lewisham, or at a GP practice.
