Connect Care now forms part of The London Care Record which enables local care teams to see relevant health and care information about a person that is held by organisations in other London areas and beyond. So for example if someone from Peckham (South East London) attends the A&E at University College Hospital (North Central London), care teams can access the information they need to treat that person quickly and safely. This could include any allergies, current medications, existing long-term conditions, and previous investigations.
Information is transferred via health information exchanges, safely and securely to provide more effective care of patients at the first point of contact. The London Care Record Programme is actively expanding within London but successful integration also with neighbouring Milton Keynes University Hospital has evidenced the initiative to also provide benefits outside of London.
