Shortlisted for four HSJ Patient Safety Awards

We are proud to announce that Lewisham and Greenwich NHS Trust’s (LGT) Adult Community Services have been shortlisted as finalists for four categories at the HSJ Patient Safety Awards 2025 in recognition of their outstanding work in improving patient safety and healthcare innovation.

The announcement comes after a rigorous judging process by a panel of esteemed healthcare leaders and patient safety experts.

The Urgent Community Response (UCR) team has been shortlisted for:  

  • Deteriorating Patients and Rapid Response Initiative of the Year
  • Community Care Initiative of the Year

The  Proactive Ageing Well Service (PAWS) team has been shortlisted for:

  • Early-Stage Patient Safety Innovation of the Year
  • Improving Care for Older People Initiative of the Year

A joint statement from Annie Curry, Head of Nursing for Lewisham Community Services and Andrew Cook, General Manager for Lewisham Community Services, said: “We are so pleased to see the inspirational work of our amazing community teams being recognised at these prestigious awards. They work innovatively and with tireless dedication to keep our patients well and independent at home, and it’s been inspiring to see how they've embraced change at every step of this journey of continuous improvement. This recognition shines a light on the incredible work happening every day in our community.”

The HSJ Patient Safety Awards celebrate those making major contributions to safer, better healthcare, from reducing harm to fostering a culture of continuous improvement. Winners will be announced at a ceremony on 15 September 2025 in Manchester, bringing together healthcare professionals from across the UK.

About PAWS team:

The Proactive Ageing Well Service supports older adults with complex needs by identifying high-risk individuals early and creating personalised care plans through comprehensive assessments. By connecting patients to community and secondary care, PAWS helps improve outcomes and reduce hospital admissions, all while promoting independence.

About UCR team:

Launched in 2021 at Lewisham and Greenwich NHS Trust, UCR aims to prevent unnecessary hospital stays by providing rapid, home-based care within two hours of referral and follow-up over five days. This innovative model ensures timely, high-quality care that keeps patients safe and out of hospital whenever possible.
