LGT Launches SWAN Model for end-of-life care

Lewisham and Greenwich NHS Trust (LGT) has launched the SWAN Model to strengthen dignified, person-centred care for patients in their final days, while supporting their families. The programme is now on Chestnut Ward at University Hospital Lewisham (UHL) and Ward 25 at Queen Elizabeth Hospital (QEH).

Originally developed by the Salford Royal NHS Foundation Trust, SWAN stands for:
  • Signs: Recognising when a patient is entering their final days.
  • Words: Using sensitive communication with loved ones.
  • Actions: Doing what matters most to the patient and family.
  • Needs: Ensuring all physical and emotional needs are met.
The model recognises that everyone in the hospital plays a part in a patient’s final journey, from doctors and nurses to chaplaincy, housekeeping, security, and volunteers.

Kate Spencer, Lead Nurse for Macmillan Palliative Care, said: "Every patient deserves to spend their last moments surrounded by comfort and love. Introducing the SWAN Model allows us to give families clear, compassionate support when they need it most."

What this means for patients and families:
  • Consistent Support: Care remains seamless across our hospitals from the end of life through to bereavement.
  • Personalised Care: Plans focus entirely on comfort, dignity, and individual wishes.
  • Memory-Making: Teams will help families create keepsakes and spend precious time together.
  • Visual Reminders: A discreet SWAN symbol near the bed space signals staff to maintain a quiet, extra-sensitive environment.
The current launches are just the first step. The Trust plans to roll out the SWAN model across all adult wards over the coming months to ensure every team is fully trained.
