Complex Needs Nursing Team
The Special Needs Nursing Team offers specialist health visiting support and special needs nursing to children and young people ages 0-19 years, with complex learning/physical needs in the borough of Lewisham.

What services are provided?
The team consists of a health visitor, paediatric nurses, learning disability and nursery nurses who work across the 0-19 years age range to support families in the following services:
- Continence
- Enuresis clinic for children with special needs
- Toilet training advice
- Early support
- Care planning
- Transition into adulthood
- Signposting to support services
- Advice and direction to respite care
- Training and education for parents, carers and other professionals
- Home visits for additional support
- Transition through the child’s school years
- Providing a special needs link throughout the borough
- Continuing care packages
Continuing Care Service
This service is for children and young people who require a specific care package due to disability, accident or illness, including life threatening or life limiting conditions where care cannot be provided by universal services alone. Through a holistic approach, we address the health and disability needs of the child or young person.
Continuing care is a funded package of care offered to children and young people who are eligible and meet set criteria. Following an assessment and panel decision based on their eligibility they will receive an individual care package that meets the child requirements.
We aim to support parents and carers as we recognise that the best place to care for a child with complex health and disability needs is in their own home. As children develop their health need and social care changes and by frequently reviewing and assessing the child it will be ensured that the right care is being provided. Both parents and child are involved in the development of the care package.

We accept referrals to the team from:

- Schools/nurseries
- Doctors and GPs
- Health visitors and school nurses
- Therapy teams
- MAPP
- Health and social care
